
Enrolment Terms and Conditions
By completing our enrolment form, you agree to the following terms and conditions.

1. Agreement to Enrolment

I agree to the registration process of this practice.

If under 16 years of age, a parent, guardian, or caregiver must sign on my behalf.

I intend to use Waiora Wellness Center as my regular and ongoing provider of general practice / GP / health care services.
-
By enrolling, I will be included in the enrolled population of National Hauora Coalition (NHC), Waiora Wellness Center’s Primary Health Organisation (PHO). My details will also be recorded in the Practice, PHO, and National Enrolment Service (NES) registers.
2. Eligibility and Entitlement

I confirm that I am eligible for publicly funded health services in New Zealand.
-
Eligibility may include being a NZ citizen, resident visa holder, eligible work visa holder, Australian resident with 2+ years stay, or a person with refugee/protected status.
-
For enrolment purposes, “residing permanently in NZ” means intending to stay at least 183 days in the next 12 months.
-
Proof of eligibility may be requested.
3. Privacy and Use of Information

I have read and understood the Use of Health and Privacy Information Statement on the website https://www.waiora.health/
-
The information I provide will be used to confirm eligibility, provide care, and meet PHO/NES/Ministry of Health reporting requirements.

Information may be shared or compared with other government agencies, but only when permitted under the Privacy Act 1993 and the Health Information Privacy Code.

Waiora Wellness Center will collect, store, and use my information lawfully and confidentially.
4. Transfer of Records

If I was previously enrolled with another practice, I consent to Waiora Wellness Center requesting my medical records.

I understand that until records are received, some services (e.g. repeat prescriptions) may be limited.
5. Fees and Payments
-
Enrolled patients may access subsidised fees; casual patients pay higher charges.
-
Fees are payable at the time of consultation unless otherwise arranged.
-
Overdue accounts may incur administration or debt collection fees.
6. Information About Enrolment

I have been informed of the benefits and implications of enrolment, including services provided by Waiora Wellness Center and NHC, along with the PHO’s contact details.
-
I understand I can only be enrolled at one general practice at a time.
-
I may change or cancel my enrolment at any time by notifying Waiora Wellness Center.
7. Surveys and Quality Improvement

Waiora Wellness Center participates in national patient experience surveys.
-
Participation is voluntary and anonymous.
-
I may decline or opt out at any time, without affecting my care.
8. Updating Information

I agree to notify Waiora Wellness Center of any changes to my contact details, address, or eligibility status.
-
I understand that not updating details may affect my enrolment and access to subsidised services.
9. Dis-enrolment

If I do not use Waiora Wellness Center’s services for a period of three years, the practice may contact me.

If I cannot be contacted, my enrolment may be withdrawn, and I will need to re-enrol.
10. General Conditions

Enrolment is provisional until identity and eligibility are verified and accepted by Waiora Wellness Center.

Waiora Wellness Center may amend these Terms & Conditions. Updates will be published on the Waiora Wellness Center website.
11. Acknowledgement
By ticking the box / signing below, I acknowledge that:

I consent to Waiora Wellness Center acting as my regular health care provider.

I accept the enrolment, privacy, and payment conditions outlined above.
12. Onboarding and Next Steps
Thank you for registering with Waiora Wellness Center.

Following our onboarding process, we will contact you within 48 hours of submission to confirm your enrolment with us.

It may take a few days to complete your enrolment with the National Enrolment Service (NES). We will contact you once this step is finalised.

If you have consented to transfer your medical records from your previous practice, this may take up to one week. However, you do not need to wait for these records before booking an appointment once your enrolment has been confirmed.

After confirmation, we will contact you by email or phone to collect any additional details and verify your identification before your initial appointment.
